Can chiropractic or acupuncture help with sciatica?

Sciatica is pain that runs from the low back or buttock down the leg, and it is one of the common reasons patients book spinal decompression with us. Dr. Hua may also use adjustments and acupuncture, depending on the exam.

Sciatica is a description of where the pain goes, not a diagnosis by itself. It usually means something is irritating the sciatic nerve or the nerve roots that form it, such as a bulging disc or tight muscles in the hip.

Dr. Hua checks your back, hips, and leg strength and reflexes to understand what is going on. Some cases need an MRI or a referral first.

What the research says about sciatica

Research on sciatica is smaller than for back pain, but some trials show spinal manipulation can help leg pain from a disc problem. Guidelines suggest manual care as part of a plan that includes exercise.

    Trial in patients with a disc bulge

    A 2006 double-blind trial in The Spine Journal compared real and simulated spinal manipulation in 102 people with back pain and sciatica from a disc protrusion. More people in the real manipulation group became pain free, and they used fewer anti-inflammatory drugs. [1]

    Added benefit when combined with exercise

    A 2014 trial in the Annals of Internal Medicine found that adding spinal manipulation to home exercise and advice gave a short-term improvement in back-related leg pain. [2]

    NICE guideline

    The UK guideline on low back pain and sciatica (NICE, NG59) says manual therapy can be considered, but only as part of a treatment plan that includes exercise. [3]

Limits of the evidence

Sciatica has several causes and some need a medical doctor or surgeon. Loss of bladder or bowel control, numbness in the groin, or leg weakness that is getting worse needs emergency care.

Questions patients ask

When is sciatica an emergency?
Go to the emergency room if leg pain comes with loss of bladder or bowel control, numbness in the groin or inner thighs, or weakness that is quickly getting worse.
Do I need an MRI first?
Not always. Dr. Hua will tell you after the exam whether he thinks imaging is needed before treatment.
